Instructions For Parents You should have read the entire document which is the Prospectus for Admissions to Sri Vani Vidyashala High School. The link below will take you to the Application Form. The admission form is only available online. There are no printed documents at school this year. Fill up the form on-line. When you "SUBMIT" the form after completion, a copy of the form will be sent to you at the e-mail address you have given. Check your SPAM folder in case your email service accidentally classifies it as such. Print the filled in form and submit (the hard copy) to the school along with the other documents listed below. Download the "Parent Questionnaire" which is attached here. You will need to spend some quality time thinking about your responses (approximately two days). Do not hurry because we really value your responses. Submission of Forms Please submit the hard copy of the completed application forms at the school for details refer Admission Calendar. What do I carry to the school for submission? The application should include all of the following documents: 1. Application Form : The completed Application that you have printed. Bring one copy for submission and keep one copy at your home. (for your records) 2. One photograph (recent) pasted on the Application Form. 3. A photocopy of the birth certificate. 4. Rs 200/- to be paid in cash at the school office as registration fee at the time of submission. 5. Proof of Residence: Please submit a proof of residence - a photocopy of a suitable document - like Ration Card*, passport, Adhaar Card, Electricity Bill, Phone bill, (At least two months latest) etc. Since we are giving preference to people staying in the neighbourhood, it is fair to ascertain that the applicants are staying in the neighbourhood. * - Ration Card – Name of the Child and his/her mother should be included in the ration card and if the child’s name is not included but you have initiated the process in the ration office, then a copy of the application submitted to the ration office and a copy of the counter-foil should be enclosed, later on you have to submit the ration card in school). 6. Copy of Caste Certificates from competent authority only for SC & ST candidates. 7. Copy of Pan card and Aadhar card of both Mother & Father. 8. You are requested to bring all the above original documents for verification along with the Xerox copies. All documents being submitted should be 'self-verified' - with the parents' signature. Payment Note: (At bottom of form). Please ignore it. We will accept cash at school office. We are unable to remove that note.
